Patient Requirement Form
- Patients must arrive at least 40 minutes before their appointment to avoid rescheduling.
- Payment (co-pay or cash) must be completed before meeting with the provider, without exception.
- To cancel or reschedule an appointment, patients must notify the office at least 24 hours to avoid a $75 late cancellation fee.
- No-show or no-call cancellations will result in a $75 no-show fee.
- Patients are responsible for updating their contact, insurance, and pharmacy information on file.
- Medication refill requests will be reviewed within 48 hours.
- Stimulant requests will require a urine drug screen at the time of script pick-up, and the patient will be notified when the script is ready to be picked up at the office.
